Selection options:

1. Select a profession and a credential number.
2. Select a profession and enter an exact last name.
   
3. Enter an exact last name and enter a first name or partial first name.
   
   

Search tips:

1. The profession (credential type) is the numbers after the dash of your full credential number.
2. The license number is the numbers before the dash of your full credential number.
3. The last name search must be an exact match.
4. The first name can be searched using just an initial or a partial name.
