11/21/2022 You have been identified as possibly having a delinquent state tax liability owed to the Wisconsin Department of Revenue. Under Wisconsin law, your occupation license or credential cannot be issued or renewed until this matter is resolved and the Department of Revenue issues a tax clearance certificate to the licensing agency.
You will need to contact the DOR by phone at 608-267-0833, or by mail at PO Box 8901, Madison, WI 53708-8901 to resolve this matter. The department’s office is located at 2135 Rimrock Road in Madison. Please call ahead to ensure staff availability if you plan to visit the office in person. Once that is resolved we will be notified. -OA-
