If your coverage isn’t through an authorized insurer, a cut-through endorsement will be required. Your insurer NEEDS to verify that your coverage is afforded through an authorized provider by checking the web site of the Office of the Commissioner of Insurance at this address: https://sbs.naic.org/solar-external-lookup/ before submission. Both forms (as well as a cut-through endorsement if applicable/required) should be submitted annually prior to expiration TOGETHER to DSPSCredSecurity@wi.gov. This information can be found on the receipt you received at the appointment, or by contacting FieldPrint directly. If you have not yet submitted fingerprints you will need to schedule an appointment at www.FieldprintWisconsin.com. Provide the Fieldprint code FPWISECURITY when scheduling your appointment to ensure they are routed correctly. Fieldprint will collect the required fees for fingerprinting services. Two forms of ID are required at your appointment: a picture ID and a secondary ID. You will be provided a list of acceptable forms of ID with your appointment confirmation.***IMPORTANT***: You must notify us by e-mail at DSPSCredSecurity@wi.gov AFTER your fingerprint appointment so that we may retrieve results. Your fingerprints will not be reviewed unless we receive notification of submission. ---em------
