7/5/18 Received form 2252, personal statement, La Crosse PD list, and receipt. Thank you. We still need:
1. 2. A copy of the police report(s) and/or criminal complaint;
3. A copy of the judgment of conviction. (*NOTE* a CCAP print out does NOT satisfy this requirement.)
If Judgment of Conviction indicates sentencing was a fine only the receipt satisfies the Sentencing Verification requirement. If additional sentencing appears we will need proof of completion of those items as well.
Upon receipt your file will be reviewed in the order received.

6/18/18 A search of the records of the Crime Information Bureau of the Department of Justice/FBI indicated pending charges and or conviction/s. Please complete Form #2252 and for EACH conviction submit:
1. A personal statement describing the facts that led to your conviction (i.e. who was involved, where you were, what happened, and why.)
2. A copy of the police report(s) and/or criminal complaint;
3. A copy of the judgment of conviction. (*NOTE* a CCAP print out does NOT satisfy this requirement); and
4. Documentation verifying that you completed the terms of your sentence (sentencing verification).
If charges were dismissed please submit proof of dismissal from the processing court. You do not need to submit any of the above items for dismissed charges.
