08/29/17: You answered NO to the questions on page two of the application, but a search of the records of the Crime Information Bureau of the Department of Justice indicated pending charges and/or conviction/s. Please complete Form #2252 and for EACH conviction submit:
1. A personal statement describing the facts that led to your conviction (i.e. who was involved, where you were, what happened, and why.)
2. A copy of the police report(s) and/or criminal complaint;
3. A copy of the judgment of conviction. (*NOTE* a CCAP print out does NOT satisfy this requirement); and
4. Documentation verifying that you completed the terms of your sentence (sentencing verification).
If you discover that any of the required information is not available after contacting the appropriate agency/police department(s), and/or court(s), please indicate this in the personal statement(s) and submit the personal statement(s) to DSPS, along with any documentation that is available. You may email these documents directly to DSPSCredSecurity@wi.gov , or fax to 608-261-7083 or mail to: "DSPS, P.O. Box 8935, Madison, WI 53708." File will be sent for CIB review once all supporting documentation has been received and all other requirements have been met.
