The Credentialing department posts status information following receipt and review of application materials.

•Fee of $8.00 CIB review fee
•Copy of the police report or criminal complaint.
•Certified copies of court documents for conviction
•Verification of compliance with all terms of each sentence, including chemical dependency assessments (if ordered by the court).
•Personal statement describing the facts that led to each offense (i.e. who was involved, where you were, and what happened), penalties imposed, and verification you completed all requirements.
•IF your conviction was drug or alcohol related, please submit a current usage statement.
•IF your conviction was a felony, we will need the names of training schools and dates you attended any training school, this can be sent via email.

But you are not eligible for a temporary work permit or the permanent license after passing the exam until this information has been approved by our Department. ***The review process of the above requested information is not started until ALL documentation is received. The review process can vary in time and can take up to or more than 30 business days if all required documentation is not initially received. All files are reviewed in the order in which received.
