02/26/2016 If you have not already done so, you need to send our certification form (Form #1576) to the previous State Board for completion. This must be received in our office directly from the previous State Board. A copy of your license is not acceptable. You must be currently licensed in at least one state.

As a requirement for licensure, you need to take and pass the Funeral Director state laws exam. You may take the exam before completing the other requirements shown on your Check Form. You have been assigned an application ID number which is shown on this Check Form. You may take the exam at any time from any computer. Please refer to the instructions posted online at http://dsps.wi.gov/Licenses-Permits/FuneralDirector/FDIRexams The test name and test password are both FSA13A (no spaces or punctuation).

8/10/2016 Congratulations on passing the exam! You may download the licensure application (Form #403) from our website at http://dsps.wi.gov/Licenses-Permits/FuneralDirector under Applications Forms. No additional fee is required. You must be employed at a licensed funeral home located either in Wisconsin or in another state.
