1/15/15 All licenses for the Barbering and Cosmetology professions will expire on 3/31/2015. If a license is issued before 2/15/2015, it will need to be renewed before 3/31/2015 by paying the renewal fee of $82.00. Please let us know if you would like the license issued now or if you would like to wait until after 2/15/2015. If you wait until after 2/15/2015, the license will be valid through 3/31/2017 and no additional fee is required. Please email your response to dspscredbac@wi.gov and include your application number found on this Check Form. Or return a copy of your Check Form along with your statement. We will not issue a license until after we hear back from you in writing.
