3-17-14 update: Also, since your application is outdated, we need further information: From 9/13 thru the present date-no more than a 3 month gap in time allowed from date of graduation, please list professional and nonprofessional activities—including capacity in which you are/were employed; INCLUDING the beginning and ending dates (month & year) of each activity. 9/19/13: Need clarification. You indicated starting as a RN in 1967 however you did indicated a graduation date of 8/1983. Please explain how you were practicing as an RN prior to graduation. You can email explanation to DSPSCREDNursing@wi.gov

9/19/13: From date of initial graduation to present date. These dates were either not accounted for or need clarification. All time must be accounted for including professional and non-professional activities. Time must be given in mo/yr format for start/end dates & include name of facility, location (city/state/country) and capacity in which you are/were employed (i.e. RN, LPN, CNA, etc.) You can fax info to 608-261-7083 or email info to DSPSCREDNursing@wi.gov
